We will need the following information:
(If you want to remove any words or numbers please state that in the notes)
(If you want the shirt to be the colors in the picture please state that in the notes)

- Mascot or Team Name
- Grandparent name (Mimi, Nana, etc.)
- Name for back (one name)
- Design Colors (Two colors only/color chart in pictures)
